During your first visit to SCC, our wonderful staff will provide you with important health forms to fill out so that we can better understand your health history.  You will also need to bring a photo ID and insurance card.  If you would like to print and fill out your forms before your appointments, we provide those on our FORMS Link.

Your payment in full is required the first visit, and your second visit will be scheduled at the front desk.

On your second visit, our doctors will share your examination and x-ray findings.  Your recommendations for care will be provided as well as other home exercises if they would be beneficial.  You will then be given your first chiropractic adjustment and therapy if needed.

Our insurance coordinator will discuss your insurance coverage and financial details.
